- To Qualify for Home Health Care you must:
Require the supervision of a Skilled Nurse
Be
under the Care of a Physician
Be restricted to leave the home due to illness or injury
- Referrals:
Referrals
can be accepted from hospitals, physicians, relatives, friends or yourself
All referrals must be approved by your physician
One
of our Registered Nurses will evaluate the patient at the home and based on the evaluation and physician's instructions we
will formulate a plan of care made to fit your individual needs
